Important Information for Your First Visit
At Olathe Chiropractic, our goal is to provide the best in patient care. To save time during your first appointment, our office has made our new patient forms available online. Simply print, complete, and bring your forms with you to your first appointment.
Please do not email your New Patient Paperwork to us before your appointment, you must physically bring it with you.
It is also extremely important to show up
15 minutes early to your appointment time if you choose not to print off the New Patient Paperwork and complete it in advance.
If you show up to your scheduled appointment time (or less than 15 minutes before your scheduled appointment time) without your New Patient Paperwork completed, you may be turned away and rescheduled to a different day and time for another deposit amount.
Those who show up late to their appointment time, even with their New Patient Paperwork completed, may be turned away and rescheduled to a different day and time as well, with another deposit amount. Our schedules are very specific, and even being a few minutes late can throw off our schedule for our patients for the rest of the day.
Parking
Both of our buildings are located in beautiful Arbor Creek Village on the Southeast corner of 159th and Mur-Len Road in Olathe, Kansas. Main parking (and handicap parking) is located on South Bradley Drive on the West side (in front of) the businesses, and there is also parking on South Brookfield Street on the East side (behind) the businesses in the residential neighborhood.
